When Care Is Needed Quickly,

the Home Shouldn’t Stand in the Way

A fall, stroke, diagnosis, or hospital discharge can suddenly mean a loved one needs assisted living or memory care. When selling the home is necessary to fund that care, Divine Transitions helps families manage the home, personal belongings, and transition logistics—while our trusted placement specialists guide the search for the right care community.

Placement Through Trusted Specialist

The right professional for every part of the transition

Divine Transitions partners with experienced senior placement specialists who help families explore assisted living and memory-care options based on care needs, budget, location, and personal preferences.

This allows each professional to focus on their area of expertise: the placement specialist guides the care-community search, while Divine Transitions manages the home, belongings, and logistics that can otherwise delay the move.

What this includes:

Connecting the family with a trusted placement specialist

Coordinating communication when appropriate

Aligning the placement and home-transition timelines

Helping the family manage the practical steps surrounding the move

Home Sale and Transition Coordination

When the home needs to fund the care

For many families, assisted living or memory care cannot be funded until the senior’s home is sold. But preparing and selling a home during a medical or family crisis can feel impossible.

Divine Transitions helps create a clear path forward. We assess what needs to happen, coordinate the necessary professionals, and help keep the process moving so the home does not unnecessarily delay the care your loved one needs.

What this includes:

  • Creating a strategy to help sell the home quickly when care can’t wait

  • Developing the fastest, most appropriate path to sell the home

  • Keeping the home sale and senior move aligned on he same timeline

  • Providing one trusted point of contact from start to finish

Personal Belongings Support

Sorting through a lifetime of belongings can feel overwhelming — especially during a major life change.

A home often contains decades of belongings, memories, paperwork, furniture, and household items. Families may be trying to manage all of this while also making urgent care decisions.

Divine Transitions helps create and coordinate a practical plan for what will move with the senior, what will remain with family, and what will be sold, donated, distributed, stored, or removed.

Handling life's possessions with care and respect

What this includes:

  • Creating a plan for personal belongings

  • Supporting downsizing and relocation decisions

  • Coordinating resources when additional help is needed

  • Reducing emotional and logistical overwhelm

Reviews

  • “She worked on our timeline — and was there through it all.” Madena supported our family through nearly a decade of transition — from helping my parents move into a safer home, to guiding us through my mom’s memory care journey after my dad passed. What stood out most was that she worked on our timeline, not hers. She was a calm, steady presence during some of our most difficult moments, and because of her, my mom is now safe and beautifully cared for.

  • “She was the calm in the middle of complete chaos.” When my mom had a stroke, I was 150 miles away dealing with my own health issues and an upcoming surgery. Madena happened to be there — she stepped in immediately, took my mom to the hospital, and didn’t leave. What followed was overwhelming — coordinating rehab across state lines, legal paperwork, facility tours, and medical transport. Madena handled details I couldn’t manage from a distance and made sure my mom was always cared for with dignity. She brought order to chaos and was a steady, trusted presence every step of the way.

About

Hi, I'm Madena.

At my core, I've always been someone who advocates for others — especially during times when life feels overwhelming and decisions need to be made quickly.

My work is deeply personal. I've supported four of my own family members through senior transitions, walking through the emotional and logistical challenges that come with navigating care, the home, and everything in between. Each experience was different, bringing its own unique challenges and decisions — something that showed me how personal and nuanced every transition truly is.

I've seen firsthand how quickly things can become overwhelming and how difficult it can be for families to manage it all on their own. Families get stuck — not because they don't care, but because there are so many moving pieces and not enough support.

That's why I created Divine Transitions for Seniors.

Today, I help families navigate the home, belongings, and logistics side of transitions — bringing structure, clarity, and calm so they can move forward without feeling overwhelmed. When circumstances change quickly and the home must be sold to fund care, I coordinate the expedited home sale and help families create a thoughtful plan for personal belongings. Families are never charged for my services.

Beyond the Business

Earlier this year a friend asked me to check on a senior couple here on the Long Beach Peninsula. When I visited and assessed the situation — a husband doing his best to care for his wife after a stroke, calling paramedics simply because he couldn't safely help her up — I realized this probably wasn't a one-off. There had to be others out there quietly struggling the same way.

So I posted on a local community Facebook group and asked a simple question — is this unusual, and if not, what can we do about it?

The response was overwhelming. 271 reactions and 104 comments in 12 hours.

Out of that conversation, I founded the Divine Connection Network.

DCN is a volunteer organization connecting isolated seniors on the Long Beach Peninsula with community support through two programs:

Neighborhood Senior Watch — neighbors keeping a caring eye on senior neighbors, so no one slips through the cracks quietly.

Adopt-a-Senior — one-on-one volunteer connections that bring consistent presence, support, and dignity to seniors who may have limited family nearby.

The Long Beach Peninsula is a uniquely beautiful and uniquely vulnerable community. Nearly half of our population is over the age of 65 — yet senior care resources here are extraordinarily limited. Seniors who need assisted living or memory care often have to leave the Peninsula entirely to find it. Families are spread across states. And the isolation that comes with that gap is real and daily.

DCN exists because Divine Transitions for Seniors exists — and Divine Transitions for Seniors exists because of what I saw when I walked through that front door.

This work doesn't stop when the business day does.
